Photograph of a peritoneal defect (hole) within the cul de sac behind the uterus. These defects in the peritoneum have been associated with pelvic pain and have been shown to involve endometriosis over 75% of the time. Therefore, I grasp the base of the defect gently with a needle nose (big bird) grasper and elevate it so as to expose the inner peritoneal lining that most often contains the endometriosis. This exposed peritoneum is then ablated (removed) preferably with a CO2 laser using ultrapulse power settings.
